East San Jose navigation center is a one-stop resource facility

The Grail Family Services navigation center in East San Jose offers case management, in addition to referrals, parent workshops, after school programming and a math and reading mentorship program. The goal is to provide residents community and social services in one place, breaking systemic barriers to care.
