Why an agent tests a change against a process that still holds the old module in memory — hot reload misses a boot-loaded shared file, module cache survives, the binary was never rebuilt — why green 'I tested it' can prove nothing, and the clean-restart, build-hash, and canary-log checks that prove the new code actually ran.
